Payment Procedures & Obligations *

Please read and check all of the below payment procedures and obligations. By checking ALL boxes below I personally agree and understand the Payment procedures and obligations.

There are NO REFUNDS on or after Feb 1st.

A $30 late fee will be applied on the 15th of the month to all overdue accounts unless payment arrangements have been made prior.

Payments are to be made in one of the following ways 1. Payment in full made at time of registration or prior to September 1st. 2. Half payment (5 months) made upon registration, & half (5 months)paid January 1st. 3. First month & june fees paid upon registration and equal monthly payments made the 1st of each month Oct-May.

I understand all competition related fees (choreography, entry fees, costume fees etc) are in addition to monthly class fees and will be paid by the requested due dates.

Photo/ Video Release *

I release all photos and videos of my dancer to be used for promotion and advertising for Abstract Dance Academy on our social media pages and or website.
